Executive Order 94-4 (23 June 1994, Arne Carlson) establishes the Governor's Ombudsman Committee for Mental Health and Mental Retardation, with the same function and same number of members (see related agencies entry).
In 2005 the Office was renamed from the Ombudsman Committee for Mental Health and Mental Retardation to the Office of the Ombudsman for Mental Health and Developmental Disabilities.
